The problem I describe, where timesyncd doesn't appear to send an NTP request for more than 10 minutes, happens consistently after a few reboots. (I only noticed this because one time I powered on the PI it had been off for a day and was still showing yesterday's date.)
I'm inclined to dump systemd-timesyncd and just use chronyd. I suspect there's some edge-case bug in it but I'm unwilling to spend my leasure time teasing it out.

I have multiple other Linux systems operating without problems and have had this DNS caching server configured the same way for years.Based on my testing I'd say that something is a bit awry with your DNS configuration, but without further details I can't offer any useful suggestions.
